The care needed is determined through an assessment that allows the program manager to create an accurate care plan based on the assessment of the individual, observations from the family, and a safety check of the individual’s living environment. This program works closely with local hospices, assisted living facilities, medical home health programs, Alzheimer’s Association, Visiting Nurses Association and local parish nurses to provide wrap-around care for each individual.

Our on-call staff handles any issues or changes in schedules that might occur.
